OptumServe COVID-19 Vaccination Requirements

Vaccines are individuals who are 65 years of age and older, OR healthcare workers, OR first responders

Vaccines will be distributed as follows:

• Monday – Those who meet the above criteria and have a birthday in January or February
• Tuesday – Those who meet the above criteria and have a birthday in March or April
• Wednesday – Those who meet the above criteria and have a birthday in May or June
• Thursday – Those who meet the above criteria and have a birthday in July or August
• Friday – Those who meet the above criteria and have a birthday in September or October
• Saturday – Those who meet the above criteria and have a birthday in November or December

Individuals 65 years of age and older must bring photo ID to verify Date of Birth 

Healthcare workers and first responders must bring work ID/badge to verify employment

Sites are open Monday-Saturday from 11 a.m. until 7 p.m. – those wishing to get a vaccine must be in line no later than 6 p.m.

Masks must be worn and social distancing requirements must be adhered to at all times

Restrooms will be available for those receiving the vaccine only

Overnight parking/waiting in line is NOT permitted
